Should the patient chart questions appear in multiple columns and/or rows on each page, or as a standard survey with questions stacked vertically?
Are there specific colors or formatting styles you want applied to the chart questions?
How should the chart questions be grouped together? Are there specific page breaks you had in mind or certain questions you would like on the same page?
If there are logic for certain questions based on previous questions, should we assume to split the chart pages accordingly?
Will your Data Processing team require a custom data format for the case form? For example, some clients require data stacked by patient chart. If so, could you let us know what formats are needed with the custom layout (CSV, SPSS, ASCII)?
For stacked files, how should we provide the data?
One stacked file, non-chart data will only be included for the first chart.
One stacked file, data will be repeated for questions that are not part of the charts.
Two data files: one with stacked chart data and the second with data from the main survey.
Are there any questions or variables that are not in the chart section of your document, but that will need to be included in the stacked file?
